For your convenience and to help us process your pet(s) in a more efficient manner we are making our various patient processing forms available online and ready to download.

By filling out the appropriate form(s) prior to dropping off or bringing in your pet, you will be helping us and yourself by speeding up the necessary "paper work" that may be required for the services that will be rendered to your pet.

	New Patient Registration	If you and/or your pet are new to the Schulhof family, you can help us and yourself by downloading and filling out the New Patient Registration form befor your initial appointment. By doing so you will help us to expedite the new patient processing.
	Hospital Drop-off Sheet	If you have scheduled to drop-off your pet for the day to be looked at by a doctor or to have a procedure done, you can download and fill out the Hospital Drop-Off Sheet prior to dropping off your pet.
	Surgical Consent Form	If your pet is coming in for a scheduled surgery such as a spay, neuter, or a dental, you can download and fill out the Surgical Consent Form prior to dropping them off for their procedure.
	Grooming Reservation Info Sheet	If your pet has a grooming appointment, you can download and fill out the Grooming Reservation Info Sheet prior to dropping them off for their appointment. This is especially helpful if you plan to drop-off your pet between 7:30 and 8:30 because the groomers may not be in yet to talk to.
	Boarding Agreement	Prior to bringing your pet for Boarding, you can download and fill out this form to save time when you drop off your pet, as well as select the services you desire. Make sure your pet’s vaccinations are up to date.
	Holiday Boarding Agreement	If you are boarding your pets during school vacations or holidays, use this form instead.

Simply present the appropriate form(s) to one of our friendly receptionists when you come in for your pet's appointment or to drop them off for boarding or a procedure.
